Our single-family home offers a spacious kitchen with a large kitchen nook, a dining room accommodating 8 persons, and a comfortable living room with high ceilings and ample natural light. The family room is filled with games, toys, and even computers, along with a cozy gas/wood fireplace. With five bedrooms, two half bathrooms, and cable TV connections throughout, our home ensures a pleasant stay. Step outside into the expansive fenced backyard with a serene lake view. The prime location allows easy access to Lake Mission Viejo facilities, shopping, dining, entertainment, and places of worship. As a provider of ongoing services for clients with developmental disabilities since 1998, South County Care II offers a comprehensive program for ambulatory adults aged 18-59. Staff Duties & Responsibilities

Duties

  • Assisting clients with self help skills: eating grooming dressing bathing and toileting.
  • Assisting clients with using different modes of communication both verbal and non- verbal (sign language communication boards pictures and body gestures).
  • Assisting clients to move freely about in the facility backyard and neighborhood.
  • Assisting clients out in the community with mobility training and making purchases.
  • Assisting clients to stay in contact with family and friends by using the phone and mail.
  • Assisting clients with household chores cooking and laundry.
  • Working on IPP objectives and data collection sheets.

PROGRAM ELEMENTS

Daily Living Skills

We provide prompt verbal and physical assistance to clients for showering, oral hygiene, grooming, toileting, using utensils, dressing, and wearing shoes.

Independent Living Skills

We encourage and prompt clients to enhance their independent living skills through daily chores. They can choose from tasks like cooking, cleaning, laundry, vacuuming, sweeping, mopping, and trash disposal.

Money Management Skills

We support clients in managing their finances, budgeting, and making purchases. Regular store visits facilitate personal item procurement. Emphasis is placed on fostering gainful employment for clients.

Community Awareness and Safety Skills

Clients receive mobility training for public bus or Access services, including address and phone number awareness, and safe crosswalk usage. They enjoy outings of their choice like movies, restaurants, parks, malls, beaches, bowling, and more.

Communication Skills

Clients are instructed in expressive language using signs, gestures, or simple words. They are provided with pictures and books to facilitate communication with others.

Socialization Skills

Clients learn to express appropriate emotions, including anger, fear, excitement, happiness, and boredom. They are encouraged to engage socially with peers, staff, and at community events to enhance their social interactions.

Health Skills

Clients are encouraged in exercise and healthy eating. Timely transportation provided for medical and dental appointments. They learn medication names and potential side effects.

Recreation and Leisure Skills

Clients choose activities at the facility: arts and crafts, puzzles, books, DVDs, music, games. Their interests are valued and encouraged to be shared.

Self Advocacy Skills

Clients' rights are read and posted during annual review. Advocacy assistance is offered. Staff ensures clients are treated with dignity and respect.

Behavior Intervention

South County Care II specializes in serving clients with severe behavior problems. A behavior plan will be provided within 30 days of placement, using positive reinforcement and enriching activities to address challenging behaviors.

In a loving and spiritual environment, we nurture clients' dreams and promote informed decision-making. Our comprehensive training and supervision take place in a family-like setting, encompassing social, recreational, and vocational activities. We actively engage clients' support system, encourage strong family relationships, and facilitate visits and transportation. Special family outings and community involvement further strengthen connections. With a focus on self-care deficits and behavioral needs, our level 4-I home is committed to providing continuity and avoiding frequent placements. Adjacent to a bike and walk trail, we foster community integration.
